Abstract


The widespread use of implants in dentistry has led to an increasing number of implant failures, such as occurs with peri-implantitis. The term peri-implantitis was introduced in the 1980s to describe a destructive inflammatory process affecting the soft and hard tissues around osseointegrated implants, which leads to the formation of a periimplant pocket and the loss of supporting bone. This paper reviews peri-implantitis models described in the literature and the available information on the treatment of peri-implantitis. In almost all studies, plaque accumulation induced peri-implantitis; few attempts have been made to induce peri-implantitis using mechanical overload. Various therapies have been advocated for the resolution of peri-implantitis and the restoration of the peri-implant tissues, including access flap surgery, debridement of the implant surface, chemical conditioning of the implant surface, bone regenerative procedures, and topical/systemic antibiotic or antimicrobial therapy. Regenerative treatment methods include the membrane technique using non-resorbable membranes, augmentation with autogenous bone, augmentation with bone substitutes or recombinant human bone morphogenetic protein-2, and a combination of membrane and augmentation procedures. While all of the described methods have resulted in acceptable bone gain, it appears difficult to achieve new osseointegration of treated implants. To date, no data exist to support specific treatment protocols.
